Failure Modes and Effect Analysis-For Processes
Description:
This workshop is designed to enable those involved in Processes (regardless of the output – service, or product) determine where the risk factors are; and what can be done about them. A structured approach is used to first create a Process Map (or Process Flow) identifying all steps, decision points, check and control points. Then using a tops down and or bottoms up approach, potential and known failures in the process are studied. The result is usually a large number of potential failures; left alone, these can be intimidating to the point of inaction. To resolve this, the concept of “Risk Priority Number”, and “Threshold Risk Level” are introduced and applied. This helps focus on high - risk elements, decide what corrective actions are required, and where in the process these may be deployed. During day 1 and part of day 2 guided learning takes place; participants get the opportunity to practice on their (real world) work processes during the rest of day 2. The workshop is conducted in an interactive learning environment with teams of 3 to 5 individuals conducting the analysis first on simulated, then real Processes.
